Bay Bluffs – Emmet County Medical Care Facility takes pride in its reputation within the community. We have been in existence since 1966 and have cared for thousands of local and area residents.
With the addition of Trillium and Lilac Lanes, and the increase in number of resident beds from 110 to 120, we now provide a roomier, more home-like atmosphere for our residents. Many other programs and amenities have also been added for our residents. Our Special Needs Unit now provides multiple private rooms and spacious areas for activities. Each neighborhood is decorated with a different color scheme that reflects the name of that area and allows our residents a way to recognize and locate where their rooms are. We also have a beautiful chapel that serves our residents with numerous weekly church services, a beautiful library, and living room for the residents to spend time with their family and friends. We have also made accommodations for your young children to visit and play.

Admissions are the lifeblood of this Facility as they augment the interesting makeup of our resident population. Naturally, there are forms, that need to be signed by the new resident and/or their responsible party, but the process is made as non-threatening and non-invasive as possible to enhance the experience of admission to Bay Bluffs.
The mandate from the Emmet County Department of Human Services Board is to accept hospitalized residents of Emmet County first, non-hospitalized residents requesting admission, and then all others. Bay Bluffs maintains a waiting list for both hospitalized and non-hospitalized potential residents.
When someone contacts the Admission Coordinator and requests a loved one be considered for placement at Bay Bluffs, they would receive a packet of forms, which need to be completed by the potential resident’s primary physician and returned to the Admissions Coordinator. The potential resident’s name is placed on the waiting list when the forms are completed and returned.
The waiting lists are reviewed whenever there is an open bed. When there is an appropriate potential resident, the printed information received is reviewed and the person is asked to participate in a face-to-face meeting to best assess their potential for admission thus ensuring them a successful admission to Bay Bluffs.

Dear Prospective Resident:
All persons seeking admission to Bay Bluffs (Emmet County Medical Care Facility) must have the enclosed forms completed and returned to our Facility prior to admission:
- A physical examination dated within five days of the expected admission. This form should be updated if the admission is delayed for any reason.
- The physician will sign his/her recommendations stating the need for nursing home placement.
- A current medication list, current diet, and medical history.
- Any treatment notes including physical and occupational therapy, as well as any order(s) for evaluations and/or treatment.
- Nursing notes and physician progress notes if applicable.
- A face sheet from another Facility if applicable and copies of insurance cards, both sides.
- Chest x-ray no more than 90 days prior to admission.
- Pre-admission screening forms, DSS-3877 and DSS-3878 or Level II from Community Mental Health.
Return the completed forms to the Admissions Coordinator in the Social Services Department at our Facility prior to consideration for admission. Forms can be faxed to (231) 526-4407.
